Once this form is submitted, a new name tag will be produced and you will be contacted by Dan's Awards and Engraving for payment. Name tags are $12 and will be $10 to ship. You may choose to pick up at Dan's Awards located at 39 Mansfield Ave in Willimantic, CT 06226. Store hours are M-F, 10am-5pm.
